Initial Patient History
When making your appointment with the office please be sure to give your email address. Shortly after your appointment is made, you will receive an email asking you to register with us. Click the link provided in the email and follow the directions. You will need your account number (HOM.XXXXX) with us in order to register, so please be sure to ask a staff member for it.
Once registered you will be able to fill out your New Patient History paperwork there. It will automatically be uploaded to your chart for your first visit with us.
